The long safety record, honestly framed
Methylene blue has been administered to humans since the 1890s and remains in hospital use today, which makes its safety profile better characterized than nearly anything else in the supplement conversation. At the low daily doses discussed in the wellness context, reported side effects are uncommon and usually mild.
A long record is not a blank check. The same literature that documents its safety documents its exceptions with unusual clarity, and they deserve to be taken seriously.
The serotonin interaction is the big one
Methylene blue inhibits monoamine oxidase, the enzyme family that breaks down serotonin. Combined with SSRIs, SNRIs, MAOIs, or other serotonergic drugs, it can contribute to serotonin toxicity, a genuinely dangerous condition. Case reports exist, primarily from surgical settings using large intravenous doses, and the mechanism applies at smaller exposures too.
The rule is simple: if you take any medication that touches serotonin, methylene blue requires a physician's sign-off first. Not a forum's. A physician's.
G6PD deficiency and other exclusions
People with G6PD deficiency, a common inherited enzyme condition, should avoid methylene blue entirely; in this population it can trigger red blood cell breakdown. Anyone pregnant or nursing should also stay clear, as should anyone with significant kidney impairment unless a doctor says otherwise.
Mild and transient effects reported at low doses include headache, mild nausea on an empty stomach, and, at higher amounts, the famous blue-green urine, which is cosmetic and dose-dependent.
Dose and quality do the heavy lifting
Most methylene blue problems in the wild trace to two errors: dosing like an aquarium and sourcing like one. Low, precise, consistent doses of verified pharmaceutical grade material sit in the well-mapped territory of the safety literature; heroic doses of industrial dye do not.
Get the grade right, keep the dose low, clear your medication list with a doctor, and this becomes one of the better-understood compounds a person can research.
